5/18/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Jashim upon arrival to the home setting. Upon arrival to the home setting, Jashim appeared to be in a good mood. QMHP-C engaged Jahsim in a discussion to determine behaviors since the last session. Mother reports he continues to struggle with being able to manage his anger when he is asked to complete certain task within the home. 2. Behavior: During the second portion of the session, Jahsim was displaying hyperactive behaviors such as him leaving the room when discussing his behaviors with his mother. 1.Intervention: QMHP-C listened actively as the mother that since last visit Jahsim had been disrespectful and displaying challenging behaviors. QMHP-C conducted family session in the living room. QMHP-C asked the mother to process with counselor on Jahsim's current behaviors. QMHP-C talked to Jahsim about the importance of being respectable and responsible in the home. 2. Intervention: QMHP-C utilized a therapeutic worksheet called “What is R-E-S-P-E-C-T”. Jahsim completed the exercises from the worksheet and counselor went over it with him. QMHP-C talked to Jahsim about learning that there is a right time to play and a wrong time to be playful. QMHP-C engaged Jahsim in a game to assist him with patience, and positive communication skills. QMHP-C processed on anger control and coping skill such as journaling and building constructive actions to direct his anger/frustrations. 1. Response: Jahsim stated that no changes needed to be made towards his safety plan. Jahsim and family was receptive to QMHP-C's suggestions for today's session. Jahsim struggled with understanding his mother while in session as he was focused on the game activity. Soon after, Jahsim indicated that he talked back and was disrespectful toward his stepfather by stomping his feet after being told to stop talking. 2. Response: Mother verbalized that he was tired of Jahsim's actions and felt that he do things to agitate him Jahsim interacted with counselor in a positive manner and was able to demonstrate positive communication.
